INSURANCE INFORMATION
Please bring your insurance card with you for appointments in the office and notify the staff of any changes, i.e. insurance coverage, address or employment. We will do our best to answer your questions and file all claims for you.
Our fees are very reasonable. Payment at the time of your office visit is appreciated. We are happy to explain all charges in detail.
Dr. Aichholz is a provider in most insurance plans in the Cincinnati area. The current exception is Group Health HMP/Anthem 100.
We are not accepting new medicaid patients at this time.
